Please print and complete the back to school forms below and bring them with you to New Student Orientation (6th graders) and/or schedule pick up (7th & 8th graders). 7th & 8th graders will not be given schedules until these forms are completed.

For students to carry medication with them (i.e., inhalers, epipen, etc), please complete the Authorization for Self-Administration form below.

Authorization for Self-administration

For students to take prescription medication during the school day from the nurse, parents must bring all medication and the completed form linked below to the nurse.
